Chronic diseases are among the most prevalent, costly and preventable of all health problems. According to the Centers for Disease Control, each year seven of every 10 Americans, or more than 1.7 million people, die of a chronic disease. The prolonged course of illness and disability from chronic diseases can result in extended pain and suffering and decreased quality of life for millions of Americans. They also contribute greatly to the alarming rise in health care costs.

These activities will focus on a proactive approach to health care management that promotes effective, personalized health care and targeted wellness strategies to improve the quality of life for those living with chronic disease.

This educational activity consists of recorded presentations from the 2010 Innovations in Wellness and Disease Management conference held in Las Vegas, Nevada on February 17-18, 2010.

Learning Objectives
At the end of these educational activities, participants should be able to

1. Identify innovative health management strategies to reduce medical expenses, achieve optimal lifestyle adherence and deliver a positive return on investment.
2. Recognize successful initiatives in wellness programs and provide insight into physician involvement, compliance considerations, and other program challenges, such as how to drive engagement and positive outcomes.
3. Discuss the most effective and efficient manner, using best available evidence and best practices, to prevent and manage chronic diseases—including obesity, diabetes, heart disease, chronic obstructive pulmonary disease, and chronic kidney disease—and their associated comorbidities.
4. Identify the influence of effective disease management and preventive care of patients with asthma, congenital heart disease, and orthopedic conditions to optimize clinical and financial outcomes.
5. Describe the maternal, fetal, and infant implications of chronic health conditions—such as diabetes—on pregnancy.
