Series Description
Integration of behavioral health services into the clinical primary care setting is often an overlooked component of population health management. The integration of these two specialties is an important care model needed to successfully manage population health and improve outcomes. This series will discuss best practices for the integration of behavioral care into a primary care setting.
Continuing Education
Earn 1.00 CME/NCPD credit for each session completed. These activities are designed to meet the educational needs of primary care physicians, PAs, nurses and nurse practitioners who are interested in learning about pertinent topics in behavioral health, including the screening, referral, diagnosis, and treatment for depression, along with the related aspects of Healthcare Effectiveness Data and Information Set (HEDIS) measures.
