Social determinants of health considerations for behavioral care providers working with the Medicaid population

Activity description

Medicaid provides health coverage to millions of Americans, including low-income adults, children, pregnant women, elderly adults, and individuals with disabilities. This on-demand e-module explores the impact of social determinants of health (SDOH) on the Medicaid population. SDOH are the conditions in which people are born, grow, live, work, and age, and significantly influence health outcomes. These include socioeconomic status, education, environment, employment, social support, and access to healthcare. Behavioral care providers play a crucial role in addressing these factors to improve health outcomes. Learners will gain knowledge and skills to identify and understand SDOH, assess individuals’ specific needs, and develop holistic care plans that consider these factors.

It is designed to meet the educational needs of nurses, nurse practitioners, physicians, PAs, psychologists and social workers.

Learning objectives

At the end of this educational activity, participants should be able to:

  • Discuss the various social determinants and how they impact health and behavior.
  • Evaluate the social and environmental factors affecting patients’ health and well-being.
  • Integrate SDOH into patient care plans to address the root causes of health issues, rather than just the symptoms.
  • Improve communication strategies to effectively engage with patients about their social and environmental challenges.
