Activity description

The use of multiple drugs to treat diseases and other health conditions is known as polypharmacy. This is a growing concern for older adults. Polypharmacy is more common among older adults and adverse drug reactions (ADRs) can occur. Medication adherence is also a major challenge for older adults who are often at higher risk for nonadherence due to poor health literacy and decreased cognitive function. This activity will discuss polypharmacy, medication adherence and safety concerns for older adults.

Target Audience

This activity is designed to meet the educational needs of nurses, social workers and other health care professionals (HCPs) who have an interest in learning about older adult issues with medication. 

Learning Objectives

At the end of this activity, participants should be able to:

  • Identify the common medications used by older adults and their potential risks.
  • Identify communication barriers with older adults, particularly in the context of medication management.
  • Analyze the physiological changes in older adults that impact medication efficacy and safety.
  • Demonstrate how to conduct a medication review with older adults that includes active listening and responding to concerns.
  • Implement feedback mechanisms to continuously improve patient care and medication safety outcomes.
  • Design interventions that incorporate practical elements to improve medication adherence and safety among older adults.
  • Discuss the role of geriatric syndrome and the impact on clinical and non-clinical members of interprofessional teams when managing older adults' medication regimens.
  • Reflect on current personal and organizational medication administration practices and their impact on safety and care for older adults.
