Palliative Care for Chronic Conditions

Activity description 

This activity consists of a recorded presentation from the "Medical Director and Clinician Leadership Forum 2024" held September 23-24, 2024 in Las Vegas, NV. The Forum was designed for medical directors and other clinical executives/leaders working in the managed health care industry.

This presentation provides an exploration of palliative care for chronic conditions beyond cancer, such as chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF) and other life-limiting diseases. It addresses the need for health care professionals to engage in proactive conversations with individuals about their end-of-life preferences and establish goals of care. Drawing from real-world examples, participants will learn practical strategies for effectively integrating palliative care into their practice to ensure that the decision-making process is person-centered and compassionate.

Learning Objectives

At the end of this educational activity, participants should be able to:

  • Identify the unique challenges and considerations associated with providing palliative care for chronic conditions beyond cancer, including COPD, CHF and other life-limiting diseases.
  • Discuss strategies to initiate and facilitate meaningful conversations with individuals about end-of-life preferences, treatment goals and care preferences.
  • Describe effective communication techniques and approaches for discussing palliative care options with patients and their families in a sensitive and compassionate manner.
