Pillars of Kidney and Cardiovascular Protective Interventions in Chronic Kidney Disease
Activity description
Chronic kidney disease (CKD) is a complex condition requiring comprehensive management that includes appropriate medication therapy and interdisciplinary care coordination. This activity will discuss kidney- and cardiovascular-protective interventions for CKD and examine the roles of risk stratification, estimated glomerular filtration rate (eGFR) and urinary albumin creatinine ratio (uACR). In addition, considerations for the use of SGLT-2 inhibitors will be discussed, including heart failure status and adverse effects. Approaches to interdisciplinary care coordination for CKD that integrates nephrologists, primary care physicians, dietitians, and other health care professionals (HCPs) will be highlighted.

Target Audience
This activity is designed to meet the educational needs of physicians, PAs, nurses, nurse practitioners, and other HCPs who have an interest in learning about kidney and cardiovascular protective interventions in chronic kidney disease.
Learning Objectives
At the end of this activity, participants should be able to:
- Identify how kidney- and cardiovascular-protective interventions in CKD are based on risk stratification or heat map using eGFR and uACR.
- Discuss SGLT-2 inhibitor-class medications for CKD, including considerations such as eGFR, uACR, heart-failure status, and risk of adverse effects.
- Describe approaches to interdisciplinary care coordination for CKD that may include dietitians, pharmacists, nephrologists, and other health care professionals.

About the presenter
Dr. Vassalotti is the Chief Medical Officer of the National Kidney Foundation (NKF) and Clinical Professor of Medicine in the Division of Nephrology, at Icahn School of Medicine at Mount Sinai. At NKF, his major focus is implementation of evidence-based clinical practice guidelines in chronic kidney disease (CKD). He led collaborations to develop the Kidney Health Evaluation for Adults with Diabetes quality measure to improve evidenced based estimated glomerular filtration rate and albuminuria testing to guide detection, risk stratification and interventions proportional to risk in the U.S. He also served as PI for an AARP-funded Kidney Health Evaluation for Adults with Diabetes to analyze quality measure satisfaction with detection, evidenced-based therapies and health equity. Currently, he serves as Principal Investigator for the Kidney Score Platform, an NKF educational project funded by the Veterans Administration Center for Innovation to improve awareness and education among Veterans with and at risk for CKD in the primary care setting. He is also a co-investigator for the Center for Disease Control and Prevention’s CKD Surveillance Project. Dr. Vassalotti typically sees approximately 40 patients per week and has over 100 publications in peer-reviewed journals.
