Activity description
Effective, efficient health care is dependent upon value-based care delivery that aligns with the Institute for Healthcare Improvement (IHI) quadruple aim: enhancing the patient care experience, improving population health, reducing health care costs and care team well-being and health equity. This conference will provide participants with innovational strategies designed to improve coordinated care delivery and achieve the IHI quadruple aim. Effective methods for capturing population risks and determining appropriate care that recognize the impact of social determinants of health (SDOH) on an patients’ well-being, health care utilization and costs will be discussed. Additionally, the increasing use of biosimilars and their ability to improve health care outcomes and costs will be examined.

Target Audience
This activity is designed to meet the educational needs of nurses, nurse practitioners, PAs, pharmacists, pharmacy technicians, physicians and social workers.
Learning Objectives
At the end of this educational activity, participants should be able to:
- Describe innovations in value-based care delivery as discussed by a panel of care system partners, including actionable ideas on how to create change within organizations to achieve the IHI quadruple aim.
- Identify strategies to achieve coordinated care through directing patients to in-network providers, engaging patients with primary care providers and reducing avoidable emergency department utilization and readmissions.
- Discuss approaches to capture population risks accurately and ensure that patients are receiving appropriate care.
- Explain the importance of SDOH data collection and its impact on patients, health care costs and utilization.
- Define key factors that influence the adoption of biosimilars across the health care industry.
