Series description and Continuing education 

Optimal Care focuses on the quadruple aim to improve patient outcomes while decreasing low value or wasted care. The Optimal Care Grand Rounds series focuses on aspects of care important to primary care providers (PCPs) and their patients. This multisession, accredited series will provide PCPs with actionable content that addresses evidence-based diagnosis and treatment approaches for many of the high-priority complex diseases frequently encountered in practice. Discussion of emerging clinical evidence and strategies to improve care will enable PCPs to gain new perspectives and increase their competency in providing care that is optimal for their patients.

Earn 1.00 CME/NCPD credit for each activity completed. This series is designed to meet the educational needs of physicians, PAs, nurses, nurse practitioners and other HCPs who have an interest in increasing their knowledge of the current state of care for many of the highly visible and complex diseases encountered in daily health care practice.
