Caring for Children in Foster Care: Managing Care Transitions and Placements

Activity description

The goal of the foster care system is to promote the well-being of children and adolescents by providing for their health, safety, stability and permanency. Nearly 75 percent of children are placed in foster or kinship care because of abuse and/or neglect by parents due to substance abuse, addiction, extreme poverty, mental illness, homelessness or transience, domestic violence and criminal activity. These children present with behavioral problems well above their peers. For children who have suffered severe abuse and neglect, foster care is an opportunity for them to heal and benefit from the resources provided to them. Yet there are many barriers to providing high-quality comprehensive health care services to children and adolescents whose lives are already characterized by transience and uncertainty.
 
After participating in this activity, health care professionals (HCPs) will have a better understanding of the child welfare system and how to coordinate care of children within the foster care system. HCPs will also gain knowledge on how to address the elevated health risk profile of children in foster care and strategies to overcome the challenges related to proper continuity of care.

Target Audience

This activity is designed to meet the educational needs of case managers, counselors, nurses, nurse practitioners, pharmacists, pharmacy technicians, PAs, physicians, psychologists, social workers, therapists and other HCPs who care for children within the foster care system.

Learning Objectives

At the end of this educational activity, participants should be able to:

  • Describe the general structure of the child welfare system and how to best coordinate care for this population.
  • Recognize the potential impact that a lack of a nurturing, stable environment during the early years of life may have on children in foster care.
  • State the role of HCPs in identifying potential complications (including physical, developmental and mental health complications as well as unmet medical needs) that may occur as a result of frequent transitions with children in foster care.
  • Discuss how to navigate proper care coordination for individuals with incomplete or unavailable health and medication histories.

About the presenter
Terry Lee, MD, is a board-certified child and adolescent psychiatrist and is the Behavioral Health Medical Director at the Community Health Plan of Washington and a University of Washington School of Medicine Clinical Associate Professor of Psychiatry and Behavioral Sciences. Dr. Lee also serves as an American Board of Psychiatry and Neurology Maintenance of Certification Committee member.

Dr. Lee has devoted much of his clinical practice to providing evidence-based clinical services, consultation and treatment to youth involved with the juvenile justice and child welfare systems. He also performs advocacy work focused on increasing efficiencies in child-serving systems of care and promoting policies that increase access to effective and cost-effective treatments. He is committed to supporting and enhancing the quality of life of disadvantaged youth with mental health concerns. He is Co-Chair of the American Academy of Child and Adolescent Psychiatry (AACAP) Adoption and Foster Care Committee, and an author of the AACAP Practice Parameter for the Assessment and Management of Youth Involved with the Child Welfare System.
