Target Audience

This activity is designed to meet the educational needs of nurses and other health care professionals (HCPs) who are interested in learning about transition planning.

Learning Objectives

At the end of this educational activity, participants should be able to:

  • Recognize the impact of disabilities in healthcare and within the community.
  • Identify the importance of timely planning for a transition from Pediatric to an Adult model of care to include none healthcare considerations.
  • Discuss how to access valuable transitional resources and support for individuals transitioning to an Adult model of care.
