Commonly seen in primary care in aging populations, heart failure can be a challenging diagnosis, especially in patients with comorbidities, and studies have shown that potential heart failure symptoms can often be missed during primary care visits. This activity will review the evaluation and diagnosis of heart failure to enable primary care providers (PCPs) to recognize this disease earlier along the heart failure continuum in all patients. It will also discuss medical treatments and management strategies that can promote improved outcomes and reduce heart failure-related hospitalization in the PCPs patient population.

Target Audience
This activity is designed to meet the educational needs of nurse practitioners, PAs and physicians who have an interest in education that will facilitate the improved diagnosis, evaluation and treatment of heart failure in their patient populations.
