Registration Types
Attendee General Registration is open to all health care professionals (excludes all industry representatives*).
Attendee Diamond Supporter Registration is available for Optum clients and Optum employees.
*Industry representatives (e.g. pharma/biotech, medical device, specialty pharmacy, home health/infusion/medication management) must exhibit/support to register.

Exhibitor/Sponsor/Supporter Registration
Registration Types
Exhibitor/Sponsor/Supporter Complimentary Registration: Valid for individuals using complimentary registrations included with their company’s Exhibit, Sponsor or Supporter fee. For this registration type, your organization must (1) have submitted either an Exhibit Space Application or Sponsorship and Advertising Application, or (2) be an OptumHealth Education Annual Supporter.
Industry* Gold Annual Supporter Extra Registration: Valid for industry organizations* purchasing a registration "in addition to" those included with their Gold annual supporter agreement.
Industry* Silver Annual or Individual Supporter Extra Registration: Valid for industry organizations* purchasing a registration "in addition to" those included with their exhibit/supporter agreement.
*Examples of industry companies are pharma/biotech, medical device, specialty pharmacy, home health/infusion/medication management, etc.

* Industry organizations must be paid exhibitors or general conference supporters to qualify for the additional full conference passes provided with sponsorship packages.
Registration Information
Attendee, Exhibitor, Supporter and Sponsor Registration includes: Conference sessions, program syllabus, continuing education recordkeeping and certificate of attendance, continental breakfasts, breaks, lunches, and networking reception.
Registration confirmation: Registrants will receive an email confirmation upon successful completion of the online registration. If you do not receive a confirmation email within 10 minutes, your conference registration may not have been completed correctly. Please request assistance at moreinfo@optumhealtheducation.com.
Cancellation policy: Send cancellation requests to moreinfo@optumhealtheducation.com. A $25 processing fee will be assessed on cancellations received by Sept. 20, 2019. Only medically verifiable cancellations will be considered for partial refund after Sept. 20, 2019.
Right of refusal: OptumHealth Education reserves the right to refuse conference registration and attendance.
Consent to use photographic and video images: Registration and attendance at or participation in OptumHealth Education activities constitutes an agreement by the registrant to OptumHealth Education’s use and distribution (both now and in the future) of the registrant or attendee’s image.
