This activity will discuss kidney- and cardiovascular-protective interventions for CKD and examine the roles of risk stratification, estimated glomerular filtration rate (eGFR) and urinary albumin creatinine ratio (uACR). In addition, considerations for the use of SGLT-2 inhibitors will be discussed, including heart failure status and adverse effects. Approaches to interdisciplinary care coordination for CKD that integrates nephrologists, primary care physicians, dietitians, and other health care professionals (HCPs) will be highlighted.

The process of advanced care planning (ACP) is widely recognized as a method to support patient self-determination, facilitate decision-making and promote better care. The responsibilities of the clinician are to utilize diagnostic tests, observe vital signs, diagnose illness, administer treatment, provide a prognosis based on treatment progress and document the patient's medical history. ACP will not succeed without effective communication between clinicians and patients, as well as family members and caregivers as appropriate. Existing barriers when engaging patients in ACP may be challenging for health care professionals. This activity will describe the role of the clinician in ACP, identify existing barriers and discuss how to overcome those barriers.
