Skilled Nursing Facilities Series: Advanced Care Planning
The process of advanced care planning (ACP) is widely recognized as a method to support patient self-determination, facilitate decision-making and promote better care. The responsibilities of the clinician are to utilize diagnostic tests, observe vital signs, diagnose illness, administer treatment, provide a prognosis based on treatment progress and document the patient's medical history. ACP will not succeed without effective communication between clinicians and patients, as well as family members and caregivers as appropriate. Existing barriers when engaging patients in ACP may be challenging for health care professionals. This activity will describe the role of the clinician in ACP, identify existing barriers and discuss how to overcome those barriers.

Medication adherence is also a major challenge for older adults who are often at higher risk for nonadherence due to poor health literacy and decreased cognitive function. This activity will discuss polypharmacy, medication adherence and safety concerns for older adults.
