Pediatric Health Care Transition: Optimizing Outcomes
Activity description
This activity consists of a recorded presentation from the "33rd Annual National Conference" held October 14-16, 2024, in Bloomington, Minn. It is designed to meet the educational needs of nurses, nurse practitioners, pharmacists, pharmacy technicians, psychologists, physicians, PAs and social workers.
The transition from pediatric to adult health care is a critical phase for individuals with chronic and complex health conditions. During this activity, we will explore the importance of involving adult care providers early in the management of conditions like severe asthma to ensure continuity of care. The goals of health care transition (HCT) and its positive impact on health outcomes, particularly for those with special health care needs, will be discussed. Participants will also review clinical processes that support HCT along with common challenges. A successful pediatric HCT model will be showcased, offering practical strategies for maintaining comprehensive care as pediatric patients move into adulthood.
Learning Objectives
At the end of this educational activity, participants should be able to:
- Discuss the need for adult providers to be involved in the care of burgeoning disease, such as severe asthma, in individuals transitioning to adult care.
- Define the goals of HCT and how this process positively impacts health care and outcomes for the pediatric population, particularly those with special health care needs.
- Identify the clinical processes by which HCT is optimally achieved, integrating relevant health care teams and optimizing care plans for individuals with complicated health issues.
- Recognize the challenges associated with HCT and the role of the provider, including discussion on release of information, guardianship and healthcare proxies.
- Review an example of a successful pediatric health care transition model and strategies for continuing care for complex pediatric patients as they become adults.

Attendance
A certificate of attendance will be provided to learners upon completion of activity requirements, enabling participants to register with licensing boards or associations that have not been pre-approved for credits.
