The Patient-Centered Medical Home: Changing the Health Care Delivery System

The PC-MH is an approach to providing comprehensive primary care for children, youth and adults. The PC-MH is a health care setting that facilitates partnerships among individual patients, their personal physicians and, when appropriate, the patient's family. The goal of the PC-MH is to improve engagement and clinical and financial results with the use of remote patient monitoring and collaborative care technologies. This session will discuss one of the most compelling concepts in health care today—how collaborative care, physician involvement, and health information technology can address delivery system challenges involving complex relationships among patients, physicians, and payers to incorporate the PC-MH and achieve industry-leading, breakthrough engagement levels; improved clinical and financial performance; and maximized program engagement through the utilization of health care data and technology.
