Integrating Case Management and Disease Management to Yield Optimal Patient Outcomes
Over the past several years, the health care industry has been struggling as a whole for effective ways to manage chronic disease and illness. Case/Disease Management (CDM) programs have emerged as a more progressive means for improving health outcomes in the chronically ill population. CDM can be best summarized as an “integrated” approach to managing health care that identifies the optimal treatment approaches for patients with a specific disease and implements processes to afford compassionate and clinical-evidence-based care that will prevent disease complications and improve the health and quality of life of a given patient or patient population.
This educational activity consists of recorded presentations from the 2010 Innovations in Wellness and Disease Management conference held in Las Vegas, Nevada on February 17-18, 2010.
